Your role and responsibilities
The Patient Coordinators are responsible for the efficient through put of cases across all departments of the practice.
Working closely with all clinical teams and The Senior Leadership Team you will be responsible for overseeing the scheduling of the day, to include timely management of emergencies and prevention of delays.
You will work closely and collaboratively with the Head of Nursing Services (HONS), Clinical Director (CD), Team Leaders and Leads across the Practice to ensure the effective delivery of outstanding patient care.
You will work enthusiastically with the nursing team to help maintain a happy and effective working environment, whilst ensuring practice and group polices are always adhered to.
This role requires strong, decisive decision making and problem solving along with clear and effective communication.
As a Patient Coordinator you would work in a small team alongside our current team of coordinators to achieve the key goals of the role.
The role is a mix between clinical and non-clinical (administrative tasks) but is based within the clinical heart of the hospital.
The administrative tasks are focused on the efficient scheduling of diagnostics, liaising with anaesthesia, theatre uses and to adapt as required.
The management of patient accommodation is also combined with the clinical aspect of assisting with the initial patient admission when required.
This is a varied and fast paced role, allowing a variation from a traditional clinical nursing role without being away from the clinical environment, therefore the position would suit an experienced RVN looking for a new challenge.
This is a full-time position Monday-Friday working 8 hour shifts between 8am-6.30pm.Weekends would be worked on a 1:4 rotation, 8 hours shifts between 6.30am-6.30pm, along with participation in the Bank holiday rota.
- Work closely with the Clinical, CD/HONS and nonclinical teams, including the attendance of regular meetings to ensure all areas of the practice work together effectively and efficiently
- Actively participate in supporting new associates to help them settle into their role and the practice.
- Support with patient coordination in an administrative and practical capacity.
- Effectively problem solve and communicate decisions when required regarding management of patient accommodation, e.g. barrier nursing, consistent monitoring.
- Demonstrate an ability to motivate others and encourage them to work effectively together, in a positive and friendly atmosphere
- Provide an efficient, courteous, and helpful service to all associates and clients in a consistent timely manner, in line with practice policy.
- Liaise between all clinical teams within the practice to always ensure effective lines of communication and appropriate cascade of information
- Maintain an overview of the hospital and ensure the highest standard of nursing and client care across the team, liaise with Nursing Team Leaders or SLT if patient care concerns are suspected.
- Ensure appropriate scheduling, throughput and accommodation of admitted cases across all departments of the practice to improve utilisation and efficiency to support timely patient treatment indicated by their individual status and business needs
- Be responsible and accountable for the delivery of effective and efficient workflow of clinical cases in line with the practices policies and values.
- Demonstrate effective communications with clinicians, team leaders, leads and members of the nursing teams and SLT to ensure efficient scheduling of emergency procedures.
- Prevent the occurrence of delays by forward planning and communicating with all stakeholders across the practice, including helping in a practical capacity to progress a patient forward within the constraints of your individual professional qualification.
