Locum Band 6 Community Physiotherapist - West Essex

We have a fantastic opportunity for a locum Physiotherapist to join a leading teaching hospital, based in West Essex. They are seeking a qualified Physiotherapist with previous MSK experience.
- _T_o_ take responsibility for providing, monitoring & developing the Physiotherapy Service within the west

Essex Community:
Helping patients to engage as independently as possible in activities which enhance their health & wellbeing.

This role supports a wide range of presentations supporting both rehabilitation and long term care pathways in the community setting. This includes seeing patients in their own homes, 24 hour care facilities and clinics. To manage a caseload of patientswith complex health & social care needs, using evidence based, clientcentered principles to assess, plan, implement & evaluate interventions
The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System supporting Out of Hospital Care to support West Essex Residents. The post holder will be working as part of the Primary Care Aligned Team (PACT) supporting2 Harlow and 2 Epping Forest Primary Care Networks with a focus on prevention of admissions, supporting safe discharge as well as provision for rehabilitation for the West Essex population.

Flexible working is required, as at times you will be expected to work across sites in our other West Essex localities depending on service needs

Position Requirements

HCPC Registered
Minimum of two years postgraduate experience
Previous clinical experience performing a similar role within a Community setting
