Among various other duties, the postholder will have direct responsibility to provide operational commissioning support for adults who draw on mental health or learning disabilities services and to work closely with the learning disability and mental health market.

They will support local community MDTs so that hospital discharges are managed in a timely way, giving additional resource to support the receiving provider to maximise the success of the discharge so that people can live in the place they call home.
