Main Responsibilities: 1. Health Promotion Coordinate the practice annual health promotion programme, focussing on national and local campaigns and priorities. Ensure the programme is circulated across the practice, on the website, with the PPG, etc. and all necessary resources made available.

Work closely with other Coordinators in particular, to ensure the health needs of patients are met in line with campaign focus


2 Patient care:
Work with identified patients to coordinate and navigate necessary personalised care requirements in line with PCSP best practice. Capture detailed information from all involved in the patients care to ensure informed decision-making is possible.

Ensure that patients have good quality information to help them make informed choices about their care. Process relevant requests from hospital discharge letters, i.e. support with palliative care, safeguarding and relevant referrals. With guidance from clinicians, arrange appointments, referrals, tests and follow up of patients, completing all relevant requests.

This will specifically include Weight management referrals. Organise annual medication reviews, ensuring appointments are made for these on GP surgeries.

Following training and completion of competency frameworks, file normal bloods Following training and completion of competency frameworks, support the ANP with agreed aspects of care for patients in residential homes.

Support practice safeguarding leads to manage child safeguarding, particularly related to children newly registered.


Within the scope of the role, help patients to manage needs, answering questions, supporting to make appointments, signposting to supporting agencies, i.e.

training, employment and benefits. Provide timely reports on patient progress and maintain excellent communication with all relevant stakeholders.

Fill in forms for benefits / insurance, printing out relevant hospital letters and EMIS summaries Support clinicians to manage Ardens Diary Recalls.

Extract coding information from clinical letters, adding to patient notes and ensuring clinical follow up where necessary. Provide support to achieve agreed areas of care in QOF, focussing on long term conditions. Lead in reviewing nurse appointments to ensure they are utilised appropriately 3. Administration and Reporting Develop and maintain tracking tools to ensure tasks are kept on track.


Support the management team to monitor key performance indicators, addressing complaints and sharing good practice to the benefit of patients.

Review SARs requests to identify 3rd party information. Support the PPG to ensure information is communicated across the practice and facilitate change where necessary. Circulate clinical and governance information / updates across the practice.


Contribute to effective information management within the team including collation, analysis and reporting of data to support PCN DES targets / specification in a timely fashion.

Support Quality and Outcome Frameworks and Local and National Targets. Participate in relevant internal and external groups to provide relevant information analysis and support. Undertake clinical and administrative audits and report back on findings.


  • Interpersonal Liaise with multi agencies to coordinate pathways of patient care. Ensure excellent communication channels with the patient and wider care system regarding patient progress. Attend all necessary meetings to support patient care requirements.
Ensure close working relationships with other PCN roles such as GP Assistant, Social Prescriber, Health & Wellbeing Coaches, etc.

Maintain constructive relationships with a broad range of internal and external stakeholders including but not limited to the Federation, GP practices, ICB, STP, Community services, Secondary Care, Mental Health and Third Sector providers.

Work closely with Managers; ensuring the appropriate level of priority is assigned to each task.

Establish communication with Practice Operations Management, PCN Manager and other managers in the wider Healthcare community to ensure that services are integrated.
