Northamptonshire Healthcare Foundation Trust NHS is delighted to announce new opportunities for Community Rehabilitation Healthcare Assistant (Band 3) in the Community Intermediate Care team based both in Kettering and Northamptonshire to support the people across the county of Northamptonshire.

Join us as part of our Intermediate Care Team, working to get people out of hospital or preventing hospital admission by playing a major part in their reablement journey, setting goals and support planning with Therapy and Nursing clinicians for patients in their own home environments.

You will be playing a fundamental part in improving the lives of people when they are at their most vulnerable.

You will be working with nurses and therapists to set goals, support patients by enabling them to maintain their independence, dignity & wellbeing, give advice & support, provide rehabilitation, carry out assessments and plan for onward referrals for longer term needs
