Description

Act as the strategic lead for the key worker service, working closely with partners to develop joint working arrangements including clear pathways and joint working protocols between the key working service and their teams.

Actively build and maintain strong relationships with a range of professionals and key decision makers across teams and settings in orderto act as a link between Health, Education and Social Care teams and ensure they respond and react to needs as required.

Develop and maintain partnership working with senior multi-disciplinary colleagues to identify, guide and refer to other services where needed e.g.

carer support services, CAMHS, short breaks provision etc.

Provide clinical supervision to the Care Navigator/s enabling and supporting them to reflect and evaluate on day-to-day practices.

As part of your supervisory responsibility you will be required to make crucial judgements that will influence how services support CYP who are at crisis stage and, therefore mitigation against risk of sexual and physical abuse, complex family issues, self-harm and suicide risk is an important consideration.

This requires the ability to make good decisions quickly by using complex analysis and selecting best options to reduce risk.

Supporting audits or surveys to evaluate the impact of commissioned services on the outcomes achieved by children and young people.
