You will have an excellent knowledge and experience in delivering aspects of care in line with you professional scope of practice.

You will contribute to the assessment and monitoring of individuals including family support and advice.

You will have good communication skills, be a team player and be organised to effectively work in partnership with health colleagues, other agencies and community stake holders.

We will provide a comprehensive induction programme and a robust supervision model. Internal and external training opportunities exist in line with the requirements of the post.
