Job Summary

  • To act as an advanced clinical practitioner working independently and in conjunction with other health care professionals
  • To assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.
  • Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
  • To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice and PCN population.
  • To provide education and training to other staff and students.
  • To lead the delivery of the Children's Immunisation program
  • To become the practice Safeguarding Lead
  • To provide supervision, teaching and mentoring to the rest of the AHP team and assist partners on the annual appraisal process of the AHP team
  • To help with the delivery of the vaccination programs

Responsibilities:


Clinical:


  • To work autonomously and be accountable for own professional actions.
  • Undertake Clinical Practice at an advanced level using expert knowledge and clinical skills
  • Deliver holistic care to people accessing primary health care services.
  • Assess, diagnose and treat patients attending primary health care services with a range of acute, nonacute and longterm medical conditions.
  • Assess, diagnose and treat patients in surgery or own home who require acute medical attention and refer to appropriate agency.
  • Directly refer patients to secondary care in acute medical need.
  • Refer patients to other medical specialities for assessment when appropriate.
  • Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for investigations
  • Interpret a range of diagnostic tests and routine clinical procedures.
  • Make critical judgements to satisfy the expectations and demands of the job
  • Advise and support other junior colleagues.
  • Work within professional guidelines and codes of conduct.
  • Work within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
  • Be an Independent Non-Medical Prescriber.

CARE AND PROGRAMME MANAGEMENT

  • Develop and manage care and provides highly specialised advice concerning care normally beyond the scope of normal clinical practice. Consult, assess, establish diagnose, implement and evaluate management/treatment plans and makes referrals as appropriate. Makes fast track referrals.
  • Communicate highly sensitive condition related information to patients, relatives and carers.
  • Initiate the process of diagnosis with patients suspected to have a chronic disease
  • Contribute to practice targets both local and national within the disease management
  • Audit outcomes of care against standards and initiate changes as necessary.
  • Undertake annual reviews of longterm conditions
  • Refer to other members of the extended primary health care team as appropriate
  • To actively lead on and implement safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escalate concerns. Be the point of contact in the practice for safeguarding matters and communicate with the social services for the safety of the vulnerable in our community
  • To lead on Children's Immunisations as directed by national set targets

STRATEGIC DEVELOPMENT OF PRIMARY CARE SERVICES

  • Contribute to the strategic development of primary care and public health services to meet patient and population needs within the practice. Work/liaise across multiprofessional and agency boundaries.
  • Work at the forefront of practice innovation. Determines how to deal with ambiguous or unique problems to develop and advance primary care services for the benefit of patients, carers and communities.
  • Recognition and involvement in Morris House Group Practice as a teaching practice and help drive the Nursing, paramedics, PA's, medical Students and GP trainees training and development.
  • Develop links within the Primary Care Network and other local and national bodies relevant to primary care clinical initiatives.

HEALTH IMPROVEMENT

  • Initiate and carry out programmes of health screening and ensures other team members are aware of health agendas and screening programmes.
  • Introduce innovation in practice to meet ongoing health need demands of the practice population.
  • Work to introduce new developments that will improve health outcomes for patients
  • Share innovation and new learning with others.
  • Support practitioners in profiling the practice population in order to initiate contribute and promote improved health and prevent disease in individuals and groups.
  • Represent individuals and families' interests when they are not able to do so themselves.
  • Make referrals to statutory and voluntary agencies as appropriate.
