We are recruiting for a Specialist Senior Community Practitioner who wishes to join #TeamIOWNHS.

This post is vital in helping to enable people living with dementia and their families/carers to optimise independence through a range of assessment and therapeutic interventions.

The aim being to enhance health and well-being, reduce risks and support engagement in meaningful activity within community settings.

This post requires the practitioner to work as part of a multi-disciplinary team within the Memory Service.

The post holder will manage a clinical caseload to deliver effective interventions within this speciality.


Interventions aim to have a significant impact on the wider economy in reducing hospital admissions and re-admissions, reducing residential placements, recommending appropriate support for domiciliary care packages as well as improving health outcomes.
