The Associate Director of Community Partnerships will: p rovide support to the Chief Operating Officer to lead the delivery of priority 4 of the NHS Lincolnshire Joint Forward Plan , Delivering Integrated Community Care; s upport delivery of workstream 1, following the Independent Review of the Lincolnshire NHS provider landscape in 2023 and improve the way PCNs, community partners and stakeholders come together to deliver integrated care; have responsibility for providing leadership and day to day direction working in partnership with key stakeholders; p rovide expert advice, guidance and leadership; focus on working in partnership to shape new pathways of care.

The main areas are leadership, s trategic Development, o perational Management, c linical Governance and quality and e ducation and development and further detail is outlined within the attached Job Description.
