Description

You will work in conjunction with our qualified Physiotherapist, Occupational Therapists, Speech and Language Therapist and Positive Behavioural Support Practitioner in the day-to-day management of participants within Calvert Reconnections.

You will be responsible for delivering specific treatment plans, contributing to assessment and review.

You will be responsible for continuous monitoring of our participants, having the knowledge to refer to the clinicians should the need arise.

In addition, you will deliver moving and handling training for the wider team.


Clinical

  • Undertake delegated clinical duties within Calvert Reconnections
  • To maintain the highest possible standard of clinical practice.
  • To support Rehabilitation Coaches and Centre Support Workers to develop their knowledge and skills and establish competencies in therapeutic techniques or use of equipment.
  • To support families and carers in gaining confidence and competence in delivering rehabilitation for transition home.
  • To assist the clinical staff in the interpretation, analysis and delivery of rehab needs and contribute to goal setting and outcome measurement.
  • To work as part of the multidisciplinary team, referring back to other team members where appropriate.
  • To attend participant centred meetings and handovers when required and represent the clinical team at meeting when appropriate.
  • To assist in the evaluation of new equipment as appropriate. To be responsible for all the equipment used in carrying out duties and adhere to Calvert Reconnections policies, including competence to use the equipment and to ensure the safe use of equipment by other team members through training, teaching and supervision in practice.
  • To deliver moving and handling training and monitor compliance, ensuring team members have the right knowledge, skills, and experience to conduct their role.
  • Work collaboratively as part of the interdisciplinary team to implement tasks/activities as part of individual and group rehabilitation programmes. This will involve active participation in centre based and outdoor activities, activities in the community, aquatic therapy and support for language/communication and positive behavioural support interventions.
  • Maintain communication with the wider team about the participants needs and goals and keep up to date, accurate and confidential participant records.
  • Provide therapy cover over weekends and evenings on an agreed rota basis.
  • Support participants with personal care if required during therapy assessment or intervention and if requested to assist support staff in delivery of personal care.

  • To be aware of the health and safety aspects of your work and implement any policies that may be required to improve the safety of your work area, including the prompt recording of incidents to senior staff
  • Maintain confidentiality with regards to the Data Protection Act for all information received.
  • Ensure that the needs and choices of the participant are listened to and always respected.

£24,500.00-£25,000.00 per year
