Description

To provide a specialist, social work service to children, young people and their families attending the Renal Unit for management of all aspects of kidney disease, with a particular focus on those with chronic kidney disease requiring dialysis and transplantation.

To promote the well-being of this group of patients and help to achieve the best possible outcome of care.

To identify needs, provide and manage welfare support for renal patients and their families in all modalities


To be a source of specialist knowledge and skills for medical and support staff where psycho-social issues are a significant factor in patient and family care.


To take referrals and make assessments of the nature of the need, offering an appropriate intervention and follow on support.

To assess and refer on as necessary to other agencies

  • Adult/child service teams (social services), local authorities, disability agencies/service providers etc.


To be present and available on the Renal Unit and in the outpatient setting to offer practical and emotional support to families.

To provide a listening service to those parents/carers facing long and difficult hospital admissions. To provide counselling in relation to financial management and help contain the emotional distress for patients and families.


Work in partnership with and act as a link between the child/family and community agencies, health professionals, schools and social services and any others involved in the family/child care to ensure awareness of concerns and any issues between organisations.


Providing assistance, advice and guidance to families about play support services, local schools, Sure Start initiatives and similar parenting programmes/schemes.
