This post provides an exciting opportunity to work closely with adults who have a Learning Disability as part of a dynamic and supportive team.

Overview

The Learning Disability Continuing Health Care Team (CHC) assess and review eligibility of continuing health care. They are responsible for the case management for individuals, over the age of 18, who have CHC eligibility.

About you

You need to be caring, kind and compassionate and demonstrate a willingness to participate in ongoing learning and development opportunities.

You will also have a key role in supporting the nursing team as well as direct contact with the people who access our service and their families.

About the team

This is a nurse led team which is dynamic and supportive. The team places a strong emphasis on effective support for all staff.

This promotes a high level of motivation and determination to provide positive outcomes for the people that access the service.

Learning and Development

You will have access to a wide range of opportunities. This includes formal training, supervision and peer support. In addition there are potential opportunities to be supported to access nurse training whilst employed by the Trust.
