Role Length:
This opening assignment is for 3 months

Enhanced DBS required for this role

A Social Work Assistant is required to provide support, information and advice and technical assistance to individuals and to develop, cost, negotiate and mediate their individual support plans as required.

To assess people who are eligible for social care support to develop their own solutions and enhance their quality of life through promoting choice and to enable individuals to take control of their support plans and resources
Key responsibilities:


  • Work jointly and alone to develop support plans with people and implement these to enable people to be healthy, safe and well.
  • Using person centred approaches, enable people to develop outcome focussed support plans that meet their identified needs.
  • Support individuals to identify potential risks and find solutions.
  • Assist people to find creative solutions to meet their identified needs.
  • Present information about the support plan, and where relevant with the service users, to other professionals and relevant panels.
  • Help people to put their support plans in place in order to meet their needs.
  • Work with all relevant people and organisations in support of a person to ensure their needs are met effectively, responding to timescales as appropriate.
  • Ensure, where necessary that all relevant information is communicated to key stakeholders/providers to ensure a person's needs are effectively met.
  • Identify safeguarding risks and instigate safeguarding procedures as required.
  • Act as a source of information and advice to team members in relation to support planning.
  • Support informal/family carers and where appropriate assess and implement a carers service in line with eligibility criteria.
  • Ensure a high level of data quality for all information recorded, ensuring that all information is timely, accurate and complete.
  • Provide advocacy for individuals in relation to the selfdirected support process and the implementation of their plans.

  • Experience of creating support plans, understanding responsibilities and task associated with arranging services within the constraints of the individual's budget.
  • Experience of assessing the needs of vulnerable people and identify risks.
  • Experience of working in a Health or Social Care setting.
  • Experience of carrying out person directed assessments with adults.

Knowledge:

  • Understanding of the social model of disability.
  • Knowledge of community resources.
  • Knowledge of legislation relevant to Social Care.
  • Understands the concept of persondirected care or the delivery outcomes to adults.
  • Knowledge of the relevant legislation, policies, and guidance.
  • Knowledge of safeguarding procedures.

Skills/Technical skills:

  • Ability to communicate effectively, both verbally and in writing, with a wide range of people and professionals at all levels.
  • Able to work in a way that empowers service users and creates opportunities for independence and development.
  • Able to negotiate and hold discussions with people, clarifying their opinions and accurately assessing information to achieve mutually beneficial objectives.
  • Able to work coherently within a team and be able to demonstrate adaptability within a team environment.
  • The ability to liaise with agencies and external partners.
  • To be able to solve problems with a positive approach.
  • To be able to work under pressure and to deadlines and prioritise task to achieve targets.
  • Ability to be selfmotivated to achieve tasks and work to a highquality standard.
  • Ability to work on own initiative.
  • The ability to use observation skills and assessment techniques to assess the issues facing adults and express them in terms of outcomes.
