If you are looking for an exciting and challenging job in the NHS why not join our Community Virtual Ward Team based out of St James Clinic, Kings Lynn.

An excellent opportunity has arisen to join our team of Care Workers to support Interim Care for patients in their own home with long term conditions, palliative care and rehabilitation needs across West Norfolk.

The Virtual Ward service runs from 08:00 - 22:00 seven days a week.

Supervision will be provided by an Assistant Practitioner with oversight of a Registered Nurse. We offer the opportunity to work in an experienced and supportive team with opportunities for ongoing professional development.

To work collaboratively with the multi-disciplinary team promoting the continuity of patient rehabilitation and care as planned.

  • Provide a high standard of supportive care to adults in their own homes.
  • To be proactive in assisting the organisation of nursing and therapy activities.

The Organisation is keen to support staff in obtaining further training, following experience as a Care Worker there will be the opportunity to join our Healthcare Assistant's the organisation has many training routes to support you in your chosen career.

Implement care for patients as defined by the individualised care plan.

Enable, encourage and / or assist, individuals (where appropriate) with personal care such as bathing, showering, toileting, skin integrity, meal preparation.

Assist / support or prompt patient in managing medication from Medication Administration Record.

Read and follow and implement support plans clearly documenting patient response, ensuring patient records are completed for each visit on SystmOne daily.
