Description

REED Business Support is looking for locum social worker, full time, experienced in working with
adults with mental health difficulties and learning disability to work with the
Mental Health Social Care Teamto facilitate discharges from Prospect Park Hospital and for Reading residents placed in out of area acute mental health hospitals.

They will work alongside our existing ward liaison team, the aim of the role is to support health colleagues to improve patient flow in the acute hospitals and to ensure discharges are safe and we meet our statutory duties under the Care Act and Mental HealthAct.

The focus will be on older adults and people with a learning disability.
Good knowledge of Care Act, Mental Capacity Act, Mental Health Act particularly s117 aftercare.
