Description

The main purpose of this role is to improve overall patient safety by pre-screening patients prior to them being booked for surgery.

The aim is to highlight the patients who need optimising pre-surgery, helping to reduce the risk of pre, peri and post-surgery complications.

We are looking to recruit an organised and methodical person to join our pre
- assessment team.


As part of your role, you will be working alongside our digital Band 4 care co-ordinator and the preoperative assessment nurses to manage a caseload of patients who are waiting for elective surgery.

This will involve inviting patients to complete their digital pre-assessment, triage of patients on the elective waiting lists, booking of pre-assessment and preoperative investigation appointments, collecting notes, and supporting with prehabilitation clinic development.


This role will contribute to improved patient safety and quality standards within the Trust by providing timely pre-screening prior to surgery.

Elements of the role will include working independently and alongside pre-operative assessment staff.

This role is a newly created role to support the trust in adhering to national guidance set by GIRFT (Getting it Right First Time)


Nationally GIRFT recommends that NHS organisations should establish Perioperative Care Co-ordination teams from April 2024, to introduce early preoperative screening, risk stratification and provide an opportunity to optimise patients' health prior to surgery.

Administrative and non-clinical work to support earlier pre-operative assessment and optimisation, this will include:
Supporting patients to complete digital self-assessment questionnaires.

Sending out digital self-assessment questionnaires.

Booking of pre-assessment and pre-operative investigation appointments under direction of the Band 4 Care Co-ordinator and pre-assessment registered nurses/team.

Collecting and tracking of case notes.


Supporting the Band 4 care co-ordinator and pre-operative assessment registered nurses/team with personalised support plans for patients on the review, monitoring and optimisation pathway, with following up about nurse led referrals to other teams and with chasing and making a note of test results.


Working with the digital Band 4 care co-ordinator and preoperative assessment registered nurses/team to develop prehabilitation workshops and digital patient information.

Ensure that all patient details are correct, inserting / amending as required.

Pull and prepare case notes for the Band 4 care co-ordinator and Digital Pre-assessment registered nurses/team to triage.


Respond to all telephone calls and enquiries politely and promptly, including the relaying of messages to appropriate staff within the Department.

The post holder must be responsive to the needs of the Unit and be able to assist, as necessary, the HCA Band 3s and registered nurses in the medical assessment of patients and following training, undertake:
