The role
Lewisham Council are recruiting to an exciting opportunity to help support some of our most complex residents and how we develop our local system and services.
The role has two focuses. The first is to hold a small caseload of highly complex and multiply excluded service users.
The post holder will enable these service users to access support and resources to meet their needs by securing coordinated input and a coherent approach from key parts of the local health and social care system.
The cases will often interact with local housing, adult social care, supported housing, substance misuse, primary care, mental health services.
The other focus of the role is to map and analyse the local system response to residents who have highly complex needs.
The post holder will establish and facilitate multi-disciplinary practices to cement joint up working as the response to complex case management across Lewisham.
Location
Whilst the service is primarily based in Laurence House, we operate a hybrid approach where you will be required to work both in the office and from home on a regular basis
What we're looking for
We are looking for someone with extensive experience working with people who have complex needs.
A good understanding of the health and social care system, interventions and service which are offered for people with complex needs, and the legislative framework which underpins these services.
An understanding of the gaps and shortcoming of these services would be helpful.
You would be skilled at working with people with complex needs, with a track record of supporting people to access services.
Experience of building multi-agency partnership would be helpful in fulfilling the role. Experience of understanding processes and system to be able to make recommendations for improvements would also be helpful.
