As part of our ongoing commitment to improve the discharge process for patients transferring to Residential and Nursing Homes following a stay in acute hospitals, East Midlands Care Association (EMCARE) are looking to appoint an additional Care Home Trusted Assessor based initially at Leicester Royal Infirmary.
You will support residents primarily at Leicester Royal Infirmary but there may be some travelling to support the other hospitals across Leicestershire.
This ensures we are part of an integrated team of Health and Social Care Staff, and will liaise with local residential and nursing homes, including visiting them as needed.
We are looking to appoint a motivated recruit who has experience and may have a nursing qualification as well as experience within the management of elderly residential care and would like to become a trailblazer to support the safer Hospital Discharge as a Care Home Trusted Assessor team part time or full time.
The primary responsibility of the Care Home Trusted Assessor is to act as an independent and unbiased assessor on behalf of care homes in relation to undertaking assessments, beyond the statutory, that need to meet the needs of the individual.
Other responsibilities include liaising with the care home to obtain information about the patient and facilitate flow of information between the hospital, the home, Adult Social Care and the Discharge team.
You will need excellent communication and influencing skills as well as a passion for placing the individual at the centre of decisions about care.
Required for the role
- Registered Nurse
- At least 5 years' experience in health or social care
Desirable for the role
- Care Home Manager including Nursing
- Worked in Elderly Social Care
- Deputy Care Home Manager

£31,000.00-£41,000.00 per year
