Diabetes Family Support Worker Paediatric - Full-time Band 4


An opportunity to expand our existing Paediatric Diabetes team to work alongside the Paediatric Diabetes team at Luton and Dunstable hospital for a Band 4 Family Support Worker.

The post holder will be involved in setting up new technology for diabetes children in the local area, and enhancing the availability of technology to those whose first language is not English.

The post is a 12 month fixed term with the expectation of continuation of funding.

Hours to be worked over 5 days (for example , with 1 weekend a month.

To work alongside the Paediatric Diabetes Team to facilitate new diabetes technology within the local community for year olds.
The post holder will be working closely with Children and Young People (CYP) to engage with their diabetes.
- be able to learn new skills and implement these.
- have good communications skills and will be able to teach IT skills to CYP and their families
- be able to provide and receive complex and sensitive information requiring tact, reassurance, empathy and persuasive skills where there are barriers to understanding.
- demonstrate a safe level of interpersonal skills when managing individuals / families and complex needs and report any concerns back to the team.

The post holder will:

- be accountable to Paediatric Diabetes Specialist Nurses (PDSNs)
- work within the Multi-Disciplinary Team (MDT)
- be responsible for their own time management.
- be motivated and flexible in areas of work
- be able to speak languages of the local community to ensure the information is understood
- work within the hospital and in non-hospital settings.
- attend children's diabetes clinics to help with downloading of pumps and use of technology and provide feedback during clinic of any issues or concerns.
- support and motivate patients and parents/ carers to improve their health and wellbeing.
- support the MDT in facilitating and implementing a range of appropriate recreational and therapeutic activities that will facilitate cognitive, social, educational and emotional development throughout patients' paediatric diabetes journey.
- visit family homes to facilitate implementation of diabetes technology.
- have the IT skills required to work with the families to resolve technology problems.
- be responsible for setting up drop in clinics to be able to download insulin pumps and blood glucose sensors.
- work with schools to enable technology use for children and young people with diabetes at school.
- drive to community settings to help children and young people and their families access technology.
- be responsible for ordering of equipment for teaching or demonstrating of technology.

participate in developments in computerised administration systems by contributing to the implementation and evaluation process. In particular relation to Diamond Diabetes System
