An opportunity has arisen for a Complex Case Care Coordinator to join our Complex Case Management Service in Hertsmere.

Hi, my name is Sandra and I am the Complex Case Care Coordinator.

I have worked in the Hertsmere team for a long time and am really proud of the great care we give to some of the most complex and vulnerable people in our community.


The complex case management service takes time to understand people's complex needs and develop personalised care and support plans, including scheduling multidisciplinary meetings, liaison with GPs and other community health services, and running a geriatrician led locality frailty clinic.


This role is approximately 50% patient facing, assessing patients and signposting them to the best services to meet their needs.

The remaining 50% is office based at Potters bar Community Hospital, where you will be helping to schedule meetings, collect information about patients on our caseload, liaising with other professionals, and compiling reports.


We sit as part of the wider integrated locality team consisting of Community Nurses, District Nurses, Community Paramedics, Physiotherapists, Occupational Therapists, and Health Care Assistants.
