Description

The successful applicant will work within the beautiful location of Malvern, Pershore and Upton, and join an experienced supportive team of Service Leads.

The role is full time, with on-call management duties to cover weekend and the out of office period during the week.

A professional health qualification and up to date registration is a requirement.

As a Service Lead you will support the realisation to build a fully integrated community offer for our patients in line with the Fuller Report.


We work closely with General Practice and other system partners to: blur the lines across local health and care organisations - integrating management and leadership to enable a more 'joined up' operational solution - ultimately benefitting our patients.

optimise the potential to jointly implement the Fuller recommendations across our PCN footprints - focussing on optimising joint resources across the PCN footprint (rather than working within organisational silos).

secure effective ways to unlock potential efficiencies in relation to holistic care provision across organisations - with the patient at the heart of everything we do.

improve patient experience and ultimately result in a more effective, efficient offer (byjointly delivering outcomes,reducing visits across organisations, and supporting the green strategy).

optimise local assets (via District Collaborative engagement) to promote local self-help solutions.
