An exciting opportunity has arisen for 2 Band 6 Primary Care Practitioner to join our Primary Care Mental Health Team working within our Primary Care Networks (PCNs) in west Birmingham.

Within BSMHFT our community mental health services are well-established and as partof the BSOL community transformation programme, we are looking to increase our primary care teams with primary care practitioner roles.

This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care.

The development of our Primary Care mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.

We are looking for enthusiastic and dynamic individuals who are able to work within a multi
- disciplinary team, working alongside GP's and other primary care professionals who strive to achieve the best outcomes for individuals within the local community.
