Salary:
£22,387.61


Holidays:25 days per year plus 8 public holidays

Job Purpose:


Under the direction of their line manager, the post holder will undertake support planning, provide one-to-one support and interventions to individuals who have mental health and other complexities and are homeless or at risk of being homeless.


About this Job:

This service has been developed in partnership with Flintshire County Council and the Housing Options Team.

  • The Link Worker will work closely with the Housing Options Team and will be based in their office as a minimum one day per week.
  • The Link Worker will provide outreach support to individuals who have mental health and other complexities who are homeless or at risk of being homeless.
  • The service will operate Monday to Friday 09:0017:00 but will have flexibility to work some evenings, early mornings as required.

Responsibilities:


  • To have commitment to and demonstrate an undertaking of equality of opportunity and its relevance to different sectors within the community.
  • Receiving and dealing with referrals from the Housing Options Team
  • Processing of approved new referrals
- following procedures for completing new assessments, risk assessments and creating individual person-centred service delivery plans.

  • The Link worker will be expected to identify the causes and actively find solutions to people's homelessness
  • Completion of individual support plans
  • Follow procedures for conducting monthly reviews or as and when required due to changes in individuals needs or circumstances, ensuring risk assessments and support delivery plans are kept up to date and relevant
  • The Link Worker is to provide intensive support to individuals in order for them to move on and/or signpost the individual to the most appropriate service within a sixweek service
  • Assisting the individual in identifying and moving into a suitable home and organising their tenancy agreement
  • Supporting and assisting the individual in activities that promote health and wellbeing, community engagement, and recovery, financial budgeting, pursuing leisure and educational goals, social skills development, management of medication and appointments, and independent living skills
  • Completing the outcomes framework and producing reports for internal/external purposes
  • Attendance at multiagency meetings to ensure the complex needs of the individuals are addressed appropriately
  • Maintaining records statistics required to monitor and evaluate the effectiveness of the service
  • Data collection and appropriate recording and presentation for monitoring and quality assurance report as directed by the County Manager
  • Follow procedures for general record keeping and information collecting, record retention, adhering to confidentiality and data protection policies
  • Implementing Adferiads' equality and diversity policy and all other organisational policies
  • Communicate clearly, effectively and professionally with individuals receiving service, Support Workers, Carers, Referrers and other Professionals via multiple formats (i.e. facetoface, telephone, in writing etc.)
  • To act in accordance with the charity's Health & Safety and Safeguarding policies and to notify the line manager promptly if there are any concerns.
  • To participate in regular supervision and appraisal and undertake any relevant training as appropriate to the role and your own personal development.
  • To work in accordance with the charity's national policies and local operating procedures and those of external regulators or professional bodies.
