The Newbury Community Older Adults Mental Health Team would like to warmly welcome an experienced, enthusiastic, caring and highly motivated Senior Mental Health Practitioner to join our Home Treatment Team Service.


The Newbury Community Older Adult Mental Health Service is a multi-disciplinary service comprised of the Community Mental Health Team, Home Treatment Team, and the Memory Clinic Service.

All teams and members work closely together to offer high standards of specialist assessment, diagnosis and treatment for older adults experiencing mental health difficulties and people with cognitive impairment and/or dementia of any age in the community setting.


You will be based at Hillcroft House which is on the West Berkshire Community Hospital site situated between Newbury and Thatcham.


The Home Treatment Team hours are Monday to Friday 9-5pm or 11-7pm, Sat and Sun 9-3pm (approximately once a month).

This post is a full time post 37.5 hours per week


Working together with your Home Treatment Team colleagues to provide assessment and co-ordinated intensive tailored care to patients in the community.

Managing a patient caseload. Undertaking comprehensive assessments in a range of settings considering the person's physical, psychological, social, and spiritual needs.

Developing and reviewing plans of care with patients in association with families and carers.

Developing and maintaining effective communication and liaison with GP's, the Local Authority and Voluntary Organisations.

Participating in Service Development to enhance efficiencies while maintaining high standard of quality of care.

Developing an area of specialist interest within the area of Old Age Psychiatry.

Supporting and supervising junior team members and students.
