We have an exciting opportunity for a Diabetes Team Lead to join a fantastic team of Diabetes Nurse Specialists.

The successful post holder will be leading on service transformation and quality improvement projects and be responsible for taking diabetes care to the next level.

This is an exciting time to join the service as there are plans for a system-wide Diabetes transformation programme.

We are seeking to appoint into a clinical Team Lead / Nurse specialist role in Diabetes.

We are looking for an innovative, ambitious, highly motivated and forward thinking nurse to help us drive transformation within service delivery.

The focus will be to deliver high levels of diabetes care to people within our Community Trust.

The Community Diabetes Service provides support, management and complex case advice for adult service users with diabetes, within Lincolnshire.

The team acts as a source of expert advice and support to GP's, Practice Nurses, Community Nurses, District Nurses and Care Homes.


You will lead the team and maintain your clinical skills in delivering diabetes advice, care and support to patients / health and social care professionals.

You will need to work autonomously within a multidisciplinary environment. You will be flexible, reliable, and approachable. Flexible working patterns will be considered as will part time hours.


To coordinate and manage the diabetes nursing team to manage the needs of these complex diabetes patients living in Lincolnshire.


To be responsible, in conjunction with the senior operational and clinical leadership team, for ensuring that patients referred to the service receive competent, evidence-based care to the highest standard that is responsive to their needs and the changing needs of the local population.

To actively participate and lead the development of the service.

Provide assessment, diagnosis, and evidence based care to this patient group.

To provide effective pathway and caseload management.

To be responsible for the development and operational management and leadership of the Lincolnshire Diabetes Service.

  • To manage performance and productivity of the teams in relation to agreed targets. To attend relevant meetings, provide reports, identify concerns and manage all aspects of this, including practitioner performance
  • To work cooperatively with partners both internal and external to deliver discharge to assess patient pathways and ensure services are efficient and safe
  • Ensure and lead on timely submission of performance data i.e. Finance, Performance Plus, providing evidence to support all domains
  • To provide leadership and operational management to a delegated workforce in line with Trust policy, whilst striving to meet the aims and values of the Trust.
  • Engage with staff to develop service, in line with good practice, research and policy to meet key standards, performance indicators and targets.
  • Manage a personal complex caseload by undertaking assessment and treatment that is evidence based within a commissioned framework. To organise this efficiently and effectively with regard to clinical priorities and use of time.
