Description

To undertake the role of the Discharge Facilitator as a specialist nurse, utilising clinical knowledge to inform decision making and clinical judgment to ensure the safe and effective discharge pathway for those patients who have finished their medical intervention or require further recovery off the acute site by working in collaboration with all relevant professional groups within the Division in order to provide a comprehensive discharge.


To provide specialist advice to the multidisciplinary team, other agencies in relation to the management of patient conditions through the assessment, treatment and review planning process in partnership with the patient/carer.


Support the development of the service to improve practice and work collaboratively across healthcare boundaries, implementing and evaluating evidence based standards, guidelines, policies, as well as ensuring all patients are coded correctly in accordance with National guidelines.
