Intermediate Care beds provide 24-hour nursing care for a short period of time for individuals who are unable to manage at home as a result of a hospital admission e.g.

Orthopaedics condition or following a fall


In addition to the care provided by nursing staff, consultant geriatrician and general practitioners (GP's), a patient can also receive treatment from speech and language therapists, physiotherapists, pharmacists, pharmacy technicians, psychiatric nurse specialists, occupational therapists (OTs) and dieticians, depending on their individual needs.


The team works closely together and develops a tailored care plan to re-able patients so they can maximize their independence.


To ensure safe and effective medicines management policies, systems and processes operate within the bedded units and across the interface with linked services.

To work as part of the multidisciplinary team to assist in improving the independence, health and wellbeing of older and vulnerable people.

To be responsible for the implementation of national and local prescribing strategies within the service.

To review diagnoses and prescribe when appropriate within STHFT and Sheffield CCG policy and clinical competency.

To ensure high quality, cost-effective prescribing and to obtain improvements in medicines management for patients.

To provide clinical medication review to patients to maximise the benefits and minimise the risks from drug therapy.

To monitor, manage and implement change to meet performance targets through working effectively as part of the team.

To provide education and training for staff and patients to maximise efficiency in medicines management.

This vacancy is advertised on a fixed term contract basis for a period of a maximum of twelve months due to maternity leave of the substantive post holder.

The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and statutory maternity provisions.

To develop, monitor and review medicines management systems within the IC bedded units to provide a safe and effective medicines management service.

To promote appropriate clinical governance arrangements in relation to medicines. Undertake risk management and ensure compliance with medicines legislation.


Ensure the IC bedded units have a prescribing policy and system in place and monitor that system to achieve a high level of efficiency, in order to reduce waste and increase patient compliance with medication.


To promote the seamless transfer of pharmaceutical care across the primary / secondary care interface by improving medicines management systems.

To audit, evaluate and develop medicines management procedures and good practice guidance.

To undertake clinical audit and research to contribute to medicines management projects.

To liaise with local community pharmacies to ensure the most appropriate community pharmacy services are available to individual patients.

To actively participate in significant event reporting analysis and investigation of medication related incidents.

To promote and facilitate the use of the Sheffield Formulary and relevant NICE guidance. To undertake audit and evaluation of medicines management for older and vulnerable people.

Write, deliver and evaluate training to clinical and non-clinical staff to maximise efficiency in medicines management.


Provide pharmaceutical advice and support in order to promote clinically appropriate, cost-effective prescribing and pharmaceutical care to older and vulnerable people.
