Pilot Home from Hospital Discharge Project Support Worker

Salary:
£22,369 FTE of 37 hours (pro rata for part time posts)


Hours:37 hours per week

Monday - Thursday 9am -5pm

Friday's 9am - 4.30pm


Contract Type:
Fixed Term until 31st March 2024


Travel expenses:
Public transport at cost or 45p/mile

South Derbyshire CVS are delighted to be recruiting staff for a pilot project that will support discharges from hospital to help ease winter pressures on our hospitals.

Working alongside the existing Derbyshire Home from Hospital Support Service team this is a great opportunity to make a difference.

You will join a team working across Derbyshire and be supported in this new role. We are open to considering secondments and job sharing.

The positions we are now recruiting for are:

  • 1 x Hospital Discharge Support Coordinator (South Derbyshire and Erewash)
  • 1 x Hospital Discharge Support Coordinator (Chesterfield, NE and Bolsover)
  • 1 x Hospital Discharge Support Coordinator (South Dales and Amber Valley)
  • 1 x Hospital Discharge Support Coordinator (North Dales and High Peak)
  • 1 x Home from Hospital Project Support worker.

Purpose of job


To support the effective running of the Derbyshire Home from Hospital Support Service, Hospital Discharge Support and South Derbyshire CVS Direct Services.

To handle (input and update) client and service data to enable CVS projects to be delivered in a co-ordinated and efficient way.


Main duties

  • To deal with referrals from agencies and the public to Hospital Discharge support and Home from Hospital, referring on to the correct Home from Hospital and Discharge Support Coordinators.
  • To specifically support the Derbyshire Home from Hospital Support Service and Hospital Discharge Support, processing referrals, inputting data and ensuring referrals get to the right Coordinator.
  • To provide support when required for general CVS and project enquiries over the phone, online or face to face; ensuring people are directed to the correct service and member of staff.
  • To record jobs/tasks and update when completed on the CRM.
  • To contact service users to find out their experience of using CVS services and to find out how satisfied they were, recording responses.
  • To provide general administrative support if required.
  • To cover general reception duties as required remotely or in person.
To support the above work the post holder will be expected to:

  • Monitor their work, keep accurate records and produce reports and statistics as required;
  • Attend staff and team meetings, staff training and organisational development days and the Annual General Meeting;
  • Liaise with staff in partner agencies;
  • Be responsible for their own health and safety and the health and safety of others who may be affected by their acts or omissions at work;
  • Comply with SDCVS policies and procedures;
  • Carry out any other reasonable duties as agreed with their Line Manager.

Qualifications:

  • Working knowledge of office software packages including Microsoft Word, Outlook & CRM
  • Understanding of confidentiality
  • Understanding of equality & diversity
  • Understanding of GDPR and data protection
