Job Title:
Physiotherapist - Integrated Home Care Service

Band:
Band 6 Preferred (Potential to look at an experienced Band 5 or a Band 7)

Location:
City Wide (Nottingham)

Duration:
ASAP for 6 months minimum

Hours:
Full or Part Time considered (Start/End Times flexible)

Job Spec:

To assess, plan and develop treatment programmes for Nottingham City patients within care homes who have a wide variety of complex physical needs, to maximise the physical and social environments to help people engage in activities of daily living and contributeto multidisciplinary treatment planning.
Determine clinical diagnosis and physiotherapy treatment indicated and to maintain records as an autonomous practitioner.
To be responsible for continuing the ongoing development of high standards of clinical practice and expertise within the service contributing to implementation of specific changes to practice and service protocols and making a professional contribution tothe work of a multidisciplinary team
To advise and support care home staff in maintaining a resident's wellbeing and engagement despite.
To manage/care coordinate a caseload of patients, and will provide advice, education and teaching sessions to providers of residential/nursing care.
