We wish to appoint a:

Social Prescriber Link Worker - Children and Young People

Salary:
£21,810-£24,905pa


Hours:
Full time - 37 hours per week

Contract - Fixed Term until 31/03/2024

We are seeking a Social Prescriber to join the CYP social prescribing team.

As a new service, we are ideally looking for someone who has a passion for seeing services thrive and is confident in working both independently and within a small team to achieve successful outcomes.

As a member of Barca Children, Young Peoples and family services you will make a real difference to the lives of others.

You will be a keen advocate of the benefits of supporting young people with health and social care needs to access support in their community by linking them to a network of groups and activities; enabling them to have improved health and wellbeing.

You will manage your own caseload and support community activity where appropriate. You will have relevant experience, be an effective communicator with a passion for excellence in your professional field. It's essential that you love what you do.
