Social Prescriber Link Worker - Children and Young People
Salary:
£21,810-£24,905pa
Hours:
Full time - 37 hours per week
Contract - Fixed Term until 31/03/2024

We are seeking a Social Prescriber to join the CYP social prescribing team.
As a new service, we are ideally looking for someone who has a passion for seeing services thrive and is confident in working both independently and within a small team to achieve successful outcomes.
As a member of Barca Children, Young Peoples and family services you will make a real difference to the lives of others.
You will be a keen advocate of the benefits of supporting young people with health and social care needs to access support in their community by linking them to a network of groups and activities; enabling them to have improved health and wellbeing.
You will manage your own caseload and support community activity where appropriate. You will have relevant experience, be an effective communicator with a passion for excellence in your professional field. It's essential that you love what you do.We want you to enjoy coming to work every day and to care about your job, your colleagues and our service users.
