The role:


This is an exciting opportunity to support the younger people of Warrington with Care act eligible needs to voice their needs, goals and aspirations for their future.


We are looking for an enthusiastic Community Care worker whose primary role will be to work with Warrington's younger people who are beginning their "Preparation for Adulthood" journey.


We are looking for someone who has experience or knowledge of Person-centred planning and support planning, to spend time with our young people and then enable Adult Social Care to develop the services needed to support them through to their adulthood and beyond.


As part of this role you will work closely with the young people, their families, support networks and other stakeholders to develop a thorough "All about me " type support plan with the voice of the young person being at the heart of it.


You will be part of an inclusive and supportive team based within the Business Development team in adult social care, working alongside social work teams and contracts and commissioning to shape and improve the services for the Warrington population.

Interviews to take place early September.

DBS clearance is required.


The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
