We are seeking to recruit a highly motivated enthusiastic and forward thinking practitioner.

The post holder will have have opportunity to build upon their existing clinical experience in a supportive environment offering ongoing clinical and professional development.


The post holder will work alongside individuals, their carers, support staff, Social Care, GMP, GMSST and other key stakeholders to ensure best person centred outcomes are achieved.

The post holder will work autonomously and collaboratively within the skilled mixed integrated tea, completing highly specialist assessments whilst developing, implementing and reviewing specialist plans.


The Learning Disability and Mental Health Liaison Practitioner will provide a comprehensive service to people with a learning disability experiencing mental illness and having needs which require a collaborative MH/LD service response.

The practitioner will work across both Pennine Care (PCFT) mental health and learning disability service areas to improve the outcome and experience of people accessing PCFT MH and/or LD services.


  • Work collaboratively across a range of disciplines to continuously promote a 'needs based' approach to all service
  • Promote an integrated approach to service
  • Develop meaningful engagement with individuals who are often referred to as 'difficult to engage' and their
  • Support local systems across the PCFT footprint proactively plan for people identified as at risk of admission (Transforming Care)
  • Support through training, consultation and advice improvements in the overall capacity of MH services to support people with learning disabilities and realise benefits from accessing those services wherever appropriate.
  • Promote both emotional and physical wellbeing for a designated caseload and as a area of improvement across all relevant service areas
