This job will be working with the Meaningful Activities Team, reporting to the team leader and responsible to senior nurse, patient experience.

You will be required to work on a rotation between the emergency floor and base wards. Weekend working/shift work is required.


The focus of the role is to work alongside the multidisciplinary team to support the needs and wellbeing of a person living with dementia and/or delirium during an acute hospital admission.


This will include supporting with activities of daily living such as washing and dressing, toileting, nutrition and hydration, using creative skills to engage with the person such as arts and crafts, music or reminiscence and also liaising with the person's family/carers and ensure patient centered care is delivered.

This is a rotational post and will including evenings and seven day working (including bank holidays).

  • To facilitate appropriate activities for a caseload of patients on specified wards within UHL who have been identified as having confirmed or suspected dementia or experiencing a delirium. This will include working with individual patients and organising group activities
  • To support patients with Activities of Daily Living which will include washing and dressing, toileting, nutrition and hydration to maintain patient's identity
  • To provide practice based training and support to staff and volunteers in delivering meaningful activities to patients
  • To work with Multi-Disciplinary Team and carers to ensure that activity provision meets the identified needs of the patient
  • To feedback observations of patients' involvement in activities to members of the multidisciplinary team. This will be written and
  • To liaise with carers, outside agencies, volunteers, hospitals and community resources etc. as appropriate to ward activity provision maintaining a professional manner at all times
  • To support and encourage ward staff to organise appropriate activities in addition to those provided by the meaningful activities staff

  • Evaluate and process allocated referrals from different ward areas in order to ensure that a person living with dementia and/or delirium receives support in a timely and meaningful way
  • Use a person centred approach to support a person living with dementia and/or delirium to have access to activities which enhance their wellbeing
