Description

To provide support and time to an allocated group of service users with severe mental health issues and support and promote their recovery whilst living in the community.


To positively promote the independent living of service users within the community, facilitate social and leisure activities for service users and to focus on their direct needs, working across boundaries of care to promote social inclusion and independence and maximise the effectiveness of STR services within available resources.


To assist in the provision of quality evidence based practice, providing advice; support and care under the guidance of the Care Co-ordinator for service users with health and social care needs, and their carers within multidisciplinary setting within a defined catchment area/locality.
