Description
The Transformation Lead's purpose is to take responsibility for the effective delivery of improvement activities within their remit.

Their role is highly visible as a senior leader and they will take a structured approach to leading large scale changes, utilising the Trust's standard approach which includes A3 thinking and QI methodology.


They will be responsible for all aspects of the delivery, including planning; project start-up and delivery; working with clinical and non-clinical leads across the Trust and linking with wider stakeholders such as GPs and patients to ensure their voice guides our transformation.


They will drive the delivery of change towards the agreed service targets and lead people through the changes required to achieve the goals set out for the piece of work, managing conflict and facilitating stakeholders to move towards a shared goal.

They will ensure the effective governance of the work, and be responsible for ensuring effective benefits management is in place to track impact of the project (across Quality, Delivery, People and Finance).

Interviews are planned for 13 March 2024 and will be face to face.


The Transformation Lead will be responsible for coordinating all elements of the programme or project life cycle and will take day to day responsibility for supporting the delivery at each stage, from inception to post-deliver evaluation.


In practice, the key elements of the role will be:

Working with key stakeholders to develop a shared vision, project/programme governance structure and plans to ensure that the piece of work is delivered effectively.

Facilitating the co-design of pathway and process changes with clinical and operational leads and service users.


Ensuring an effective approach to managing the people/social elements of the programme of work, utilising approaches such as Bridge's Transitions model, Kotter's change model or ADKAR to help where appropriate.

Bridging the gap between organisational vision/direction and practical implementation: translating what is required from an organisational perspective and making it deliverable and meaningful for the front-line clinical/operational teams

Leading through others to ensure that the changes are owned by the people who will continue to work in the services at the end of the project or programme


This role is a key senior member of the Transformation team and will be expected to take a leadership role within the team to support the Associate Director of Transformation in the successful delivery of the team's objectives in support of True North and Breakthrough Objectives.
