Job summary


The post holder will be expected to have the skills, knowledge and experience to manage a first contact caseload independently with clinical debrief, supervision and mentorship being provided by the lead GP and FCP.

For less experienced clinicians this role offers the opportunity for development through stage 2 and with established clinicians being encouraged to advance their skills in musculoskeletal injections, prescribing and investigations/imaging.

Main duties of the job

  • Provide clinical expertise, acting as firstcontact physiotherapist and making decisions about the best course of action for patients' care (including in relation to undifferentiated conditions). This will involve seeing patients, without prior contact with their GP, in order to establish an accurate diagnosis and management plan.
  • Provide the link between primary, community and acute services, when the patient is managed via primary care ensuring an integrated care pathway for individual patients that meets their needs.
  • Progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of particular tests diagnostic services such as xrays and blood test, and interpret and act on results to aid diagnosis and the management plans of patients.
  • Communicate effectively between primary care services/GP practices and other relevant care providers. Educate on the role of the FCP, its impact on referral patterns and patient care pathways.
  • Develop, agree and deliver programmes of supported patient selfmanagement, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions
  • Foster links across the healthcare landscape from communitybased resources to secondary care.

PURPOSE OF ROLE
