Description

The Community Review is a warm and friendly Team responsible for carrying out planned and unplanned reviews of care packages that have been put in place for citizens to meet their assessed needs, within the relevant timescales and in line with Safeguarding observations.

The team reviews and updates care plans as necessary, with the aim of keeping citizens as independent as possible for as long as possible, whilst at the same time maintaining their safety and reducing any risks that may be identified.

We are looking for individuals who are experienced in carrying out assessments, care planning and case management in accordance with the statutory requirements of the Care Act and who have experience of working for a local authority in this type of role.

The worker will be required to attend the office and to complete home visits in Nottingham City at least 2 days per week, though some home based working is permissable.

Experience of using liquid logic would be preferable.

A knowledge and commitment to safeguarding and promoting the welfare of vulnerable adults

Ability to analyse information and make justifiable decisions to resolve problems.

Ability to form and maintain appropriate relationships and personal boundaries with vulnerable adults

Awareness of the needs of disadvantaged groups and of equality of opportunity in service delivery.

Experience in assessing situations which are not straightforward and an understanding of when to escalate issues to management.

Has a practical understanding of physical, emotional and attitudinal barriers faced by vulnerable adults and disabled people.

(HCC) To protect the safety and welfare of vulnerable adults, families and their carers, within the 'Making Safeguarding Personal' principles.

Ability To Work As Part Of A Team And On Own Innitiative

An understanding of social care, and health and their underpinning values and specifically the Care Act.

Knowledge of legislation, regulations and codes of practice, eg Safeguarding/Care Act/Mental Capacity/ Valuing People

Self Motivated, Flexible & Resourceful

Trustworthy and reliable


DBS IS NEEDED FOR THIS ROLE
Monday to Friday £16.32 per hour
