An exciting development opportunity has arisen to join our multi-disciplinary team as a band 4 discharge coordinator. There are 2 inpatient wards so we are looking at one per ward


You will be an integral member of the MDT and take on an overarching role in supporting patient flow through the system with the aim of reducing the length of stay for all patients.

The role of the discharge coordinator will be to actively and independently monitor the multi-disciplinary management and discharge plan for all patients to ensure efficient, timely discharges.

You must therefore have excellent communication skills and be confident to work on many levels within the MDT as well as external community organisations.
