We are excited to announce that we are recruiting another Band 4 Assistant Practitioner to join our Community Intermediate Care Centre.
This post is to support the Discharge to Assess Team.This post isfundamental in supporting clinical delivery and flow across the system in aiding people to regain independence and function with the view to determine long term discharge destinations.
The post holder will work as part of a designated clinical team delivering health care that focuses on the direct needs ofthe patient. They will carry out specific clinical tasks and responsibilities as identified by registered practitioners and
accept clinical responsibility for a caseload where appropriate.
In addition, the post holder will develop and maintain knowledge, skills and competences, under supervision, related to the
role of Assistant Practitioner.
The assistant practitioner will work as part of a multi-disciplinary/integrated team. The role will work across Physiotherapy
and Occupational Therapy boundaries providing support to both professions
The service works as an integrated MDT primarily including allied health professionals, nursing and HCA staff and social workers.
Your role will be to provide holistic assessments to an identified group of patients and be able to follow them up to enable them to reach their rehabilitation potential.
The environment is 'ward-based' on a community site, however you will be expected also to conduct visits to patient's homes to support discharge planning and follow up support.
Ideally, we are looking for someone who can demonstrate that they are practising at an enhanced level and have achieved competencies equivalent to that of an Assistant Practitioner.
You will be well supported in this new role by experienced therapists and via our robust supervision, training and appraisal structure.
