This is an excellent development opportunity for a dynamic individual to gain new experience in a 12 month fixed term/secondment role supporting complex discharges on the Paediatric wards.


An organised, self-managing individual with a good track record of working independently and as part of a team your tenacity, methodological approach and excellent communication skills will ensure that you are successful and feel a strong sense of achievement from co-ordinating across professional groups to achieve timely discharge for the young people on our wards.


As part of a recently established team you will play a key role in organising and supporting discharge planning, liaising with Children's Social care and Children and Adolescent Mental Health Services to ensure clear plans and appropriate escalation to reduce delays for patients once they are medically fit.

You will work closely and have support from the Paediatric Senior Management, as well as the Positive Behaviour Support team
- both of which have oversight of the Children and Young people we have admitted with complex mental health, physical health, and social issues.
