Overall Purpose:

As a member of the inpatient team, to contribute to creating an environment where the highest standards of palliative care and support are provided to patients and their families, during the changing phases of their illness.

Expectations of the Role - Accountable for own practice and for training and delegating to voluntary staff. - Receive instructions on patient condition and care required. Also receive instruction about support relatives/friends are requiring.

  • Carry out routine patient care duties at own discretion.
  • Use initiative when responding to the support that patient relatives/friends require. Nursing supervision always available.

Key Result Areas/ Responsibilities Patient and Client Care:

  • To assist with patients personal care when toileting and bathing both in the bathroom and for patients who are nursed in bed.
  • To assist as necessary with eating and drinking.
  • To assist with mobility and personal comfort.
  • To assist with total care of terminally ill patients, family and friends.
  • To work effectively as a member of a team under direction of nursing staff.
  • To undertake delegated tasks to contribute to safe and smooth running of service.
  • To assist with 'last offices' after patient death.
  • To assist with support given to bereaved relatives/friends.

Communication:

  • To develop skills in communicating with patients and families/friends.
  • To communicate relevant information to rest of team concerning patient and family and respect confidentiality.
  • To participate in MDT discussions.
  • Overcome barriers to communication when dealing with patients with cerebral related problems, learning difficulties, sensory deficits and those totally unable to communicate verbally, requiring use of lite writers and other aids.
  • Form relationships of trust with patients/families and engage them in way that respects their views, autonomy and culture.
  • Respond to patients/families emotional needs in a sensitive and supportive manner.
  • Offer informal bereavement support to patients relatives.
  • Communicate information to patients' relatives when in their own home

Escort Duty:

  • Act as escort for highly dependent patients in the minibus daily, on transport to and from their own homes to the unit and to hospital appointments.
  • Fulfil this role adhering fully to minibus policy.

Responsibility for Financial and Physical Resources: - For regularly handling patients' valuables and possessions. - For regularly handling money donated to the charity and passing this to official fundraising team. - For ordering CSSD supplies and maintaining stock levels.


Human Resources:

  • To communicate patient/families care needs to volunteers, delegating to them as appropriate.
  • For demonstrating and explaining duties to new volunteers.
  • For being a first point of contact for dealing with volunteer concerns/queries. Policy/ Service Development and Delivery
  • Work within agreed procedures and policies of the Hospice Trust including responsibility under Health and Safety at Work Act.
  • To report all accidents/incidents to nurse in charge.
  • Responsibility to ensure that inpatient unit is well maintained, kept tidy and complies with health and safety guidelines, ensuring the safe use of equipment and storage of materials.
  • Contribute to implementing hospice Clinical Governance initiatives in recognition of the Hospice Trusts commitment to quality assurance. Documentation of Information
  • Responsible to keep accurate, up to date written information of care, support, given to patients/families to assist ongoing planning, evaluation and development. The post holder may occasionally hear sad or distressing information or deal with upset relatives/visitors.


The postholder may have to deal with service change, working at home and using different working methods; for example, during the Covid 19 pandemic, changes to job roles were required in order to keep the Hospice services running.
