Following a redesign of services, we are in an exciting position to be able to recruit an Associate Practitioner as part of our Dementia Services in the Basildon & Brentwood locality.

We are seeking an Associate Practitioner to work in partnership with Community Dementia Nurse Specialists and other Senior MDT members to offer assessment, treatment and provide care and support to people with a diagnosis or suspected diagnosis of Dementia and their Carers within community settings and/or the patient's own home.

This is an important role working as part of a team with a friendly atmosphere and strong work ethic. This post qualifies for HCAS 'Fringe'.
