Responsibilities:


  • Assess patient needs and develop individualised care plans.
  • Coordinate with other healthcare providers to ensure that the patient receives comprehensive care.
  • Monitor patient progress and adjust care plans as needed.
  • Provide education and support to patients and their families.
  • Communicate with insurance companies to ensure that the patient's care is covered.
  • Maintain accurate records of patient care plans and progress.
  • Ensure that all relevant laws and regulations are followed.

Salary:
£19,062.00-£19,474.00 per year


Schedule:

  • Monday to Friday
  • Weekend availability

Supplemental pay types:

  • Loyalty bonus

Experience:

- providing care: 3 years (required)


Licence/Certification:

  • Driving Licence (required)

Work Location:
Hybrid remote in Lowestoft, NR33 9QS
