Responsibilities:
- Assess patient needs and develop individualised care plans.
- Coordinate with other healthcare providers to ensure that the patient receives comprehensive care.
- Monitor patient progress and adjust care plans as needed.
- Provide education and support to patients and their families.
- Communicate with insurance companies to ensure that the patient's care is covered.
- Maintain accurate records of patient care plans and progress.
- Ensure that all relevant laws and regulations are followed.
Salary:
£19,062.00-£19,474.00 per year
Schedule:
- Monday to Friday
- Weekend availability
Supplemental pay types:
- Loyalty bonus
Experience:
- providing care: 3 years (required)
Licence/Certification:
- Driving Licence (required)
Work Location:
Hybrid remote in Lowestoft, NR33 9QS
