Description
To assist in the provision of an efficient administrative service for the IHDT and Hospital to Home. Act as a point of contact for all users of the Hospital to Home. Service as determined by the IHDT.

The post holder will be expected to work on their own initiative but seek guidance where they feel this is appropriate from the IHDT coordinator and other Team members.

The post holder will be expected to manage the communication and information processes for IHDT, Hospital to Home, liaising with patients and carers, support workers, wards and therapy staff.

The post holder will be required to work within Trust policies and procedures.


Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.
PRIMARY DUTIES & AREAS OF RESPONSIBILITY
Communication.

  • To receive and communicate information for users within IHDT & Hospital to Home. This
will require effective and close liaison therapy staff, support workers and IHDT.

  • Single point of contact for internal and external agencies
  • Team Leaders
  • Hospital to Home Support workers
  • Matrons/Ward managers
  • Command Centre
  • Medical Secretaries
  • Training Department
  • Performance Information
  • Community hospitals external agencies.
  • Plymouth City Council
To support and monitor the booking of patients

  • Booking Patient slots
Work to the Trust values - Put patients first, take ownership, Respect others, be positive, Listen, learn and improve. Adhere to Trust policies and procedures, e.g. Health and Safety at Work, Equal Opportunities etc.

To support the IHDT coordinator and HTH OT / PT leads to ensure capacity is utilised to its maximum.

To liaise with Advanced Therapy Support Practitioners and Therapy staff to
determine available resources and support senior team leaders to plan work accordingly.

To help support the use of an electronic patient scheduling system

To keep an up-to-the-minute record of patient bookings to ensure accurate
information is managed and relayed to relevant staff.

To ensure all relevant administrative information is up-to-date and available for reference, to include timetable.


To assist and support processes which allow rescheduling of sessions not required by patients ensuring these slots are reallocated to the team.


Work closely with IHDT coordinator and HTH lead to ensure that patient administration functions are carried out and data recorded in a timely manner.


Take responsibility for working with the IHDT coordinator and Team to establish systems and processes to ensure communication across the Trust supports improved HTH utilisation.

To maintain a log of issues / actions on a daily basis.

To ensure daily communication with operational management team regarding patient flow.

To ensure escalation of concerns is undertaken to the IHDT coordinator, HTH lead, IHDT lead.

To work flexibly to meet service requirements.

Ordering of stationary as required for both HTH and IHDT.
