We have a team of 450 care and support workers who are committed to making a positive difference in their lives.

We have an in house Positive Behaviour Support Team, an Administration team and a team of Operations Managers who all work together to deliverthe best care and support possible.

We have always been rated "Good" by the CQC and are looking for a special person to join our team who is ambitious for adults who have a learning disability and/ or Autism, someone who has a dynamic, positive and creativeapproach to help guide and support our skilled, experienced and committed team to achieve "Outstanding".


Job Purpose
This is an exciting new role working with the Registered Manager and senior management team.

You will be responsible for the day to day co-ordination of our assessment and referral processes, ensuring the appropriate placement of service users.

The roleentails working closely with commissioners, liaising with Multi Disciplinary Teams including Psychiatry, Community Nursing and Social Work teams to ensure that we assess referrals to identify suitable individuals whose needs can be met by our service and forwho suitable accommodation options can be identified.


Duties and responsibilities

  • Be the first point of contact for all referrals
  • Gather and collate information on all referrals
  • Provide timely communication and responses
  • Effective record keeping and completion of monitoring tools to track progress
  • Liaise with professionals from a wide range of disciplines and families and other supporters within agreed timescales
  • Provide appropriate information confidently about the service we provide and the potential opportunities we can create
  • Filter referrals at Stage 1 for further assessment (stage 2)
  • Gather information and liaise with General Manager to proceed with each case
  • Provide regular updates for the General Manager
  • Support in designing the shape and cost of services and developing the proposals for commissioners
  • Carry out on site assessment visits at clients home, hospital, other supported living placement for example
  • Attend meetings in person or online as needed
  • Collate and provide information for the General Manager to support in the package negotiations
  • Liaise with service heads to identify allocated leads
  • Lead on setting up mobilisation meeting with our nominated service leads and present case to the team
  • Liaise with finance and Ops on agreed packages and service design
  • Lead in handover of information and contact details to the nominated Ops and PBS leads
  • Follow up on any outstanding queries to support mobilisation team
  • Carry out any other task reasonably expected to ensure smooth processing of referrals and commencement of placement.
  • Flexibility is important as this is a new role and will evolve over time to mee the changing needs of the service
  • Background in Nursing/ Adult Social Care or similar experience
  • Excellent IT skills
  • Ability to work on own initiative and manage own workload
  • Excellent orgasnisation skills
  • Experience of working with a broad range of disciplines
  • Experience of working in Adult social Care / NHS/ CCG Autism and / or Learning Disabilities services
  • Understanding of Positive Behavior Support
  • Excellent reading and report writing skills

  • Empathetic able to understand the perspectives of all persons involved in the process and show consideration for the worries, issues and concerns experienced when going through significant changes in a persons life understand the challenges faced by operationalstaff.

Where will role be based


You will be based at our office in Enfield however will be required to attend clients homes and/ or community placements and/ or hospital / residential placements to carry out assessments and attend meetings in locations in and out of London as needed.
