Description
Clinical Participate in and undertake triage to ensure smooth progress of patients through the system.

Comprehensively assess patients with complex physical, mental and social care needs using advanced clinical examination skills, detailed history taking and biometric monitoring and evaluation skills.

Receive patients with undifferentiated and undiagnosed health care problems and initiate and conduct clinical investigations as appropriate. Analyse signs and symptoms, laboratory tests and other measures of function, in formulating a diagnosis.

Perform a medication review and prescribe and change medications as required in accordance with national and local guidance. Prescribe following a full holistic assessment and diagnosis for an acute condition.

Exercise independent and delegated prescribing rights in accordance with current and legal and ICB criteria, and within the clinical situation.

Have an understanding of the combined impact of all patients' conditions on their health and social care needs in order to improve health outcomes and quality of life.

Develop short-term individualised treatment and management plans using evidence based practice and proactively monitor patients and review care plans. Use advanced skills and expert knowledge to identify subtle changes in condition during this period and take appropriate action.

Exercise a high degree of clinical autonomy and professional judgement, treating patients independently within the service, taking an asset based approach in supporting and managing the handover of patient care to other community based services.

Identify and plan preventative measures and anticipatory care needs.

Identify at risk patients and manage appropriately in accordance with national and local policy and procedures.

Act as an advocate for the patient at all times to ensure their individual needs, preferences and choices are taken into consideration.

Professional Utilising own professional knowledge and skills, act as a resource for other members of the team. Empower colleagues and patients and demonstrate the ability to facilitate effective clinical practice based on best evidence.

Possess a clear understanding of legal and ethical issues, professional accountability and responsibility.

Work and maintain high standards of professional care in accordance with current NMC Code of Professional Conduct, Scope of Professional Practice and ethical considerations.

Take personal responsibility for keeping professionally updated and registered with the NMC. Keep accurate, contemporaneous documentation and care plans in accordance with NMC Information Governance, local and national standards.

Collaborate with the Team Manager and promote the development and effectiveness of the service utilising a multi-skilled team approach. Maintain effective channels of communication both within the service and across the wider health and social care economy. Promote and maintain the quality and standards of the services in accordance with organisational and Trust clinical governance requirements. Facilitate a culture, which promotes and maintains best practice utilising current and best evidence.
