The postholder will work under the direction and supervision of the LeadCommunity Nurse or delegated Registered Learning Disabilities Nurse to assistwith the provision of and access to Primary Care for People with LearningDisabilities, to contribute to a reduction in health inequalities.

THIS POST IS FIXED TERM/SECONDMENT FOR 6 MONTHS DUE TO FUNDING
