Cleveland Clinic London are looking to recruit a Practice Transition Coordinator to join our team.

This is a full-time role with the following shift patterns: Monday - Friday, 09:00-17:00, onsite and remote working involved.

The Practice Transition Coordinator will perform a variety of duties to support consultants with the efficient and seamless transition of patients across to Cleveland Clinic London and will be responsible for the coordination of patient appointments and activity relating to the patient visit such as liaising with patients, scheduling appointments, investigations and dealing with all related correspondence.


The role will also require effective and sensitive communication with patients to provide a world class service and patient experience.

In addition, the Practice Transition Coordinator will have excellent communication skills and be able to engage with other clinicians, general practitioners, and patients in order to build and relationships and the practice.

Over time it is anticipated that the role will evolve to include general operational/service management responsibilities within the respective Institutes.

What will your duties include?- To act as point of contact for patients, consultants, and referrers for the Institute.

To organise and manage the smooth transition of existing patients under the care of named consultants across to the Cleveland Clinic London from their previous practice.- To facilitate and organise the effective transfer of all patient documentation records such as patient care records, medical notes, correspondence and letters between consultants, clinicians, and patients.- Serve as the primary lead for training other CCL caregivers on scheduling cardiac testing, procedures, and outpatient appointments. Other duties assigned.


What we need from you?- Medical terminology qualification (AMSPAR) is desirable.- Extensive experience in the field of a practice manager or senior medical secretary is essential.- Experience working in a private healthcare setting is essential.- Experience of using Electronic Medical Records Systems is essential.- Experience of private practice management software is desirable.
