Job Purpose:

As part of the Hospice at Home Team, work with patients, carers and registered nurses in the delivery of end of life care.

To provide safe, effective and responsive nursing care that meets the physical, spiritual, emotional and social needs of patients and their families/carers.

As a Healthcare Assistant you will work directly and alone with patients and families in their own homes, on a shift basis.

You may also work with other care agency staff or in residential/nursing homes.

Contribute to the day to day operational management and ongoing development of the Hospice at Home service by undertaking a range of additional responsibilities, appropriate for level of competence as delegated by the Hospice at Home Clinical Nurse Specialist Band 6.


Main Duties and Responsibilities:
At all times actively promote patient and family/carer involvement in care Under the direction of the district nurse and on call nurse, in accordance with written guidance and procedures, undertake agreed ongoing clinical activities with designated patients


These may include:

Delivering, monitoring and documenting the patients progress in the District Nursing notes and electronically onto S1 Administering medicines as per Marr Chart via oral or topical route to designated patients for symptom management (this includes administration of controlled drugs orally) Provide personal care, pressure area care, mouth care etc.

Provide psychological / emotional support for the patient and family.

Use mobile working laptop to report handovers to the Hospice at Home Clinical Nurse Specialist the following morning Verbally report handover to the Clinical Nurse Specialist in Hospice at Home (when mobile devices are not working) Have the ability to liaise with a multi-disciplinary team Discuss the role of Chaplain and Complementary Therapist and make recommendations for referrals to Clinical Nurse Specialist Human Resources To be a positive role model for Community Palliative Care Services To share knowledge and skills to upskill the generalist Health care Services and Carers Maintaining own professional development and requirement to take part in yearly appraisal Clinical and Practice Governance Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transferred electronically must be undertaken with regard to the Charitys Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes The post holder must adhere to infection control policies and procedures Undertake mandatory training and any other training relevant to the role as required by ARHC The post holder must participate in clinical and other audits as required Provide patients and their families /carers with information on standards they should expect from the team Participate in clinical supervision on a regular basis The post holder is required to participate in relevant emergency preparedness process for their team.


Adhere to lone working policy and devices used Health and Safety Attends statutory training with respect to lifting, fire safety and any others required Reports any defect in equipment immediately to the person with overall responsibility at that time.
